Social Worker (Community Living Center)

This position is located within the Community Living Center (CLC) at the Central Alabama Veterans Health Care System in Tuskegee, AL.
The Social Worker (Community Living Center) is a member of an interdisciplinary team, and is primarily responsible for completing psychosocial assessments, completing the psychosocial portions of the care plan and the Minimum Data Set (MDS).
She/he is also responsible for facilitating the Interdisciplinary Treatment Team meetings, emphasizing culture transformation, including resident centered care planning.
Major duties and responsibilities include but are not limited to:
Conducts psychosocial assessments to determine needs of Veterans and family members; Provides psychosocial treatment to a wide variety of individuals from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds; Works with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group, and family counseling skills; Participates in a discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate; Provides consultation/education to Veterans and families on community resources, advance directives, and VA benefits;Provides consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment; Provides orientation and coaching to new social workers and social work graduate students; Acts as liaison between VA and community resources, to include marketing and public relation duties as needed; Participates in committee assignments, as deemed necessary by the service chief; Maintains data and statistical compilations to comply with Joint Commission, Veterans Health Administration (VHA), CAVHCS, and Social Work service policy and procedures;Assists patients and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness, and death; Documents social work interventions and activities in patient's clinical record and ensures appropriate hand off when transferring of patient care; and,Performs all other duties as assigned.

Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
