Registered Nurse (Case Manager)

Duties include but not limited to:
Implements the Caregiver Support Program specific to his/her area of assignment and insures compliance and collaboration with local facility and the Veterans Administration Central Office Caregiver Support Program initiatives. Participates in direct caregiver and Veteran contacts to identify and assess caregiver stress burden and the development and application of directed clinical treatment interventions. Coordinates and integrates findings from needs assessments, inter-rater reliability assessments and program evaluations and incorporates into learning activities. Demonstrates leadership in patient advocacy, in protecting the health, safety and rights of patients and caregivers. Identifies and takes actions to resolve system/process constraints contributing to ethical dilemmas. Functions as a role model in assuming responsibility and accountability for any and all actions. Makes recommendations concerning the resources available and identifies risk areas, including budgetary, staffing, supplies, equipment, and availability of data and information. Implements an educational plan to meet changing program or service needs for self and others. Incorporates current trends in clinical practice or delivery models/environments and the knowledge needed by staff to remain current and to be adequately prepared for anticipated changes in caregiver issues. Provides education and in-service training to VAMC employees, Veterans, families and caregivers. Recommends and implements changes at the facility level as it relates to the caregiver support program to meet those standards. Uses the group process to identify, analyze and resolve caregiver problems. Advises and collaborates with interdisciplinary teams throughout the medical center on caregiver issues. Creates educational tools, develops programs and implements training focused on specific caregiver needs/issues. Collaborates with patients, caregivers, healthcare team members and healthcare leaders in policy and program development, implementation, and evaluation as well as in the delivery of care. Provides hands-on support to group work and training of the PACT team members in the development and facilitation of such groups. Works effectively with others by resolving conflicts and assisting caregivers and other to cope with stressful situations. Develops and fosters relationships with community partners to assist with system navigation, maintain collaboration and coordinate services. Develops a data monitoring system to measure the efficacy of the program. Initiates and leads interdisciplinary committees, task forces, and groups to improve caregiver support within the facility. Conducts or participates in research findings or facilitates special projects designed to validate and improve healthcare delivery related to caregiver support. Presents and/or publishes and outcomes of caregiver activities as appropriate. Assists the nursing staff with assessment, implementation, and evaluation of new approaches, procedures and standards of patient care and caregiver support based on the best available evidence and research. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Preferred Education:
BSN or higher degree preferred. Reference:

This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potential periods of continued walking, standing, sitting, and lifting. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations and able to shift priorities based on needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. The incumbent may occasionally be exposed to patients who are combative secondary, but limited to psychiatric or delirium disorders.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
